Instructions / Help

First, select a network directory to begin your search. The PPO Networks directory option refers to providers for employer group and union medical plans. The Workers' Compensation and Dental Network directory option refer only to those specialty networks.

Next, select your state from the map or dropdown list to the right.

After selecting a state, the network options for that state will appear. Simply choose the network you wish to search. For current members, check your medical ID card for the network logo to select the appropriate network. Note that if you select a Superien Health Network affiliate, a new window will open that will automatically redirect you to that network's provider search page.

Note that results are limited to the state selected. If you would like to search an area that borders multiple states, you may need to search each state to ensure that you receive the desired results.
